Community + Mobile Outreach

Our mobile and community outreach services are designed to meet you where you’re at. By making it easier to access harm reduction and healthcare, we aim to reduce the barriers that make it more difficult to take care of your health.

Oxford, Elgin + Middlesex Counties

Our Counterpoint Needle and Syringe program is also delivered via mobile outreach in Oxford, Elgin, and London-Middlesex counties. These services are free, confidential, and accessible via telephone and email.

Addiction Medicine Consult Service

RHAC’s peer harm reduction workers can connect with patients who use substances, experience addiction, or are managing withdrawal in-hospital. Connections are made by physician referrals during inpatient stays at Victoria Hospital and University Hospital. This program is a partnership with London Health Sciences Centre (LHSC) and CMHA Thames Valley Addiction and Mental Health Services (TVAMHS).

H.O.M.E. Program

The Health Outreach Mobile Engagment (H.O.M.E) program provides primary care, basic needs, and harm reduction supports directly to individuals experiencing homelessness or housing insecurity. Services are delivered two days a week from a converted city bus at locations across the city of London. The H.O.M.E. bus can provide:

• Medical care (triage and assessment, episodic care and prescribing, wound care, flu shots, treatment for infections)
• Harm reduction support (equipment, supplies, services, and information)
• HIV/AIDS and hepatitis C testing
• Support navigating healthcare and social service systems
• Housing support
• Provision of basic needs (as available, e.g., food, water, clothing, hygiene products)
• Referrals to community services and supports 
• Peer support and referrals to Recovery Community Centre

The H.O.M.E. program is a partnership between London Intercommunity Health Centre, London Cares, CMHATVAMHS, LHSC, and Regional HIV AIDS Connection Services.
